What insurance plans do you accept?

We accept a wide variety of health care plans.  Please click on the Insurance Information tab at the top. 

How do I request a prescription refill?

You can easily request a prescription refill through the Patient Portal or by calling your local pharmacy. 

What hospital is Rochester Primary Care affiliated with?

Rochester Primary Care is an independent Primary Care office and is willing to work with any hospital or provider. In addition, we do admit patients to both Crittenton Hospital and William Beaumont Hospital-Troy directly. 

Are you accepting new patients?

Absolutely! We welcome new patients and look forward to assisting you with your health care needs. 

How do I contact my provider after hours?

We have a provider on call 24/7. To contact them after hours, simply call the main line at (248) 759-5460 and select the option to be connected with the provider on call. 

 

 

How long should I fast before a blood test? 

The amount of time you need to fast for will vary depending on the test. For most tests, you need to fast for 12 hours. Please do not consume anything but water or black coffee for 12 hours leading up to the test.  

Can I drink coffee if I am fasting before a blood test? 

You can consume black coffee (without cream or sugar) before a blood test. 

Tip 

Water hydrates your body and makes your veins plumper and more visible. Stay hydrated before your test. Try drinking several glasses of water right before the blood draw to make it easier for the medical professional to find a vein. 

 

 

What is a Patient-Centered Medical Home (PCMH)?

The Patient-Centered Medical Home (PCMH) model is an approach to providing comprehensive primary care for children, adolescents, and adults. The PCMH is a health care setting that facilitates partnerships between patients and their personal physicians, and when appropriate, the patient's family. 

This definition was laid out by the American Academy of Family Physicians, the American College of Physicians, the American Academy of Pediatrics, and the American Osteopathic Association in the 2007 Joint Principles for the Patient-Centered Medical Home. This defines critical principles within the PCMH model:

  • Access to a personal physician who leads the care team within a medical practice
  • A whole-person orientation to providing patient care
  • Integrated and coordinated care
  • Focus on quality and safety

Through the medical home model, practices seek to improve the quality, effectiveness, and efficiency of the care they deliver, and to ensure that the activities within the practice are focused on meeting patient needs first. The PCMH model seeks to foster a relationship of trust between the care team and the patient and to actively engage patients as partners in their health care.
